Calculate

eGFR estimates may be less accurate in extremes of muscle mass, amputees, or acute kidney injury. Confirm with clinical context.

Formula

eGFR = 142 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^−1.200 × 0.9938^Age × (1.012 if female)
κ = 0.7 (female), 0.9 (male) | α = −0.241 (female), −0.302 (male)
ScrSerum creatinine (mg/dL)
AgeAge (years)
κSex-specific creatinine threshold (0.7 F / 0.9 M)
αSex-specific exponent (−0.241 F / −0.302 M)

When to Use

CKD staging, medication dose adjustment, pre-contrast imaging assessment, diabetic nephropathy monitoring.

Clinical Pearls

  • The 2021 CKD-EPI equation removed race as a variable — use this in all patients.
  • eGFR <60 for >3 months = CKD; stage G3a or higher warrants nephrology co-management.
  • Adjust medication doses (e.g. metformin, NSAIDs, contrast agents) based on eGFR.
  • In India, lower muscle mass may lead to lower creatinine and overestimated eGFR — interpret with clinical context.
